Procedure for Issuing a Replacement Student ID Card (KTM) due to loss, damage, or for students who have never received a KTM:

1. Students must upload a photo with a red background while wearing the UIR alma mater jacket to apply for a replacement KTM using the following method :
   Students must send an SMS to 92962 with the format:
   » DAFTAR UIR KTM NPM NO_HP_PENGIRIM
   » Example: DAFTAR UIR KTM 231011234 0823XXXX (enter the sender’s phone number)
   Notes:
   » NPM= Student Identification Number
   » NO_HP_PENGIRIM= The phone number must be the same as the number used to send the SMS
   » Only Telkomsel or Indosat provider cards can be used
   » The phone number must start with 08 (without spaces or punctuation)
   » The sender’s phone number must match the data in SIKAD
   » If the phone number is different, please update your Profile in SIKAD.
   
   
2. After uploading the photo, the student must pay the replacement KTM fee of IDR.100,000 at Bank Unisritama (in accordance with the Rector’s Regulation).
   
   
3. The replacement KTM can be collected at the DPP Academic Section (Migas Center Building) after receiving a WhatsApp notification stating that the replacement KTM has been printed and can be collected 7–14 working days after payment.
   
   
4. The collection of the replacement KTM cannot be delegated to someone else.
